Los antihistamínicos con poca frecuencia pueden causar efectos adversos cardiovasculares relacionadas con su anticolinérgicos y anestésico local (quinidina-like) de las actividades. Taquicardia, palpitaciones, cambios en el ECG, arritmias, hipotensión y la hipertensión han sido reportados. Aunque estos efectos son poco frecuentes y generalmente se limita a la sobredosificación o ingesta accidental de las situaciones, los fabricantes y algunos médicos recomiendan que el tratamiento con antihistamínicos ser administrado con cautela en pacientes con enfermedad cardiovascular, hipertensión y/o el hipertiroidismo.
