A pesar de narcóticos (opioides) agentes analgésicos en general son metabolizados por el hígado, insuficiencia renal puede alterar la eliminación de estos agentes y sus metabolitos (algunos de los cuales son farmacológicamente activos), lo que resulta en la acumulación del fármaco y el aumento del riesgo de toxicidad. La terapia con opioides debe ser administrado con cautela y se inició en la reducción de las dosis en pacientes con función renal significativamente deteriorada. Las siguientes dosis debe ser ajustada en base a la respuesta individual en lugar de un fijo a su horario de dosificación.
