Los receptores de angiotensina II (AR) antagonistas pueden causar insuficiencia renal en los pacientes cuya función renal depende de la actividad del sistema renina-angiotensina-aldosterona. Además, hipotensión sintomática puede ocurrir en individuos susceptibles, que puede comprometer la función renal y de la perfusión miocárdica. En pacientes con insuficiencia cardíaca congestiva grave (CHF), el tratamiento con AR antagonistas se ha asociado con oliguria y/o azoemia progresiva y, raramente, insuficiencia renal, isquemia miocárdica, y la muerte. La terapia con AR antagonistas debe iniciarse con precaución en pacientes con problemas graves de CHF, especialmente cuando va acompañada de volumen y/o depleción de sodio. En los pacientes que sufren un deterioro en la función renal, la interrupción de la AR antagonista de la terapia no se requiere generalmente siempre hay mejoría de los síntomas de la insuficiencia cardíaca y deterioro de la función renal es bien tolerado. Hipotensión transitoria no es una contraindicación para continuar el tratamiento con AR antagonistas, ya que la terapia puede generalmente ser reinstalado sin dificultad después de que la presión arterial se estabiliza.
