Los diuréticos tiazídicos pueden ser ineficaces cuando la tasa de filtración glomerular es baja (filtrado glomerular < 25 mL/min) debido a que no se espera que se filtra en el túbulo renal, su sitio de acción. Además, los diuréticos tiazídicos disminución de la TFG y puede precipitar la azotemia en la enfermedad renal. Los efectos acumulativos también se puede desarrollar debido a que la mayoría de estos fármacos se excretan sin cambios en la orina por filtración glomerular y secreción tubular activa. La terapia con diuréticos tiazídicos debe ser administrado con cautela en la reducción de las dosis en pacientes con insuficiencia renal. Si la función renal se hace cada vez peor, como se indica por el aumento del BUN o creatinina sérica, una interrupción o discontinuación de la terapia con tiazidas deben ser considerados.
