Con la excepción de fosinopril, inhibidores de la ECA (y/o sus metabolitos activos en algunos casos) son principalmente eliminados por el riñón y puede acumularse en pacientes con insuficiencia renal. Los inhibidores de la ECA también puede empeorar la función renal en algunos pacientes bloqueando el efecto de la angiotensina II mediada por vasoconstricción arteriolar eferente, lo que disminuye la filtración glomerular. La terapia con inhibidores de la ECA deben ser administrados con precaución en pacientes con historial de insuficiencia renal, particularmente aquellos con enfermedad renovascular. Los pacientes con moderada a severa insuficiencia renal requieren generalmente menor o menos frecuente la dosis y pequeños incrementos en la dosis. Además, una reducción de la dosis o la suspensión de cualquier concomitantemente diuréticos administrados pueden ser útiles. Fosinopril probablemente no se requieren ajustes de la dosis a menos que la función hepática también es afectada de manera significativa.

En los pacientes con bilateral estenosis de la arteria renal o estenosis de la arteria renal en un riñón solitario, inhibidores de la ECA puede reducir la perfusión renal a un nivel críticamente bajo. La función Renal debe vigilarse estrechamente durante las primeras semanas de terapia.
