Ciertas condiciones se han identificado como causas de la reducción de aclaramiento de la teofilina. Ellos son la edad (neonatos y lactantes < 1 año, así como en pacientes de edad > 60 años) y las siguientes enfermedades concurrentes: edema pulmonar agudo; insuficiencia cardíaca descompensada; cor pulmonale; fiebre (>= 102 grados durante 24 horas o más, o menor elevación de la temperatura por períodos más largos); influenza; no tratada o no controlada, hipotiroidismo; enfermedad del hígado, cirrosis o hepatitis aguda; reducción de la función renal en los niños < 3 meses de edad; la sepsis con fallo multiorgánico; y choque. La terapia con theophyllines debe ser administrado con cautela en pacientes que se presentan con uno o más de estos factores de riesgo, y la dosis debe ser adecuadamente reducida para evitar la toxicidad. Un seguimiento más intensivo de suero de las concentraciones de teofilina puede ser requerido. La toxicidad es más probable que ocurra cuando los niveles superan el 20 mcg/mL. Los casos graves, a veces sin previo aviso, han llevado a arritmias cardíacas, convulsiones intratables, y de la muerte.
