El uso de fenotiazinas se ha asociado con toxicidad hematológica, incluyendo agranulocitosis, trombocitopenia, eosinofilia, anemia aplásica, púrpura, granulocitopenia y anemia hemolítica. Leucopenia leve se ha producido con frecuencia con grandes dosis durante largos períodos, pero en general ha sido reversible a pesar del tratamiento continuado. Desde thiothixene está relacionada estructuralmente con la piperazina fenotiazinas, los fabricantes recomiendan que no se utilice en pacientes con historial de discrasias sanguíneas. La leucopenia y la leucocitosis, que generalmente son transitorios, han sido reportados con el uso de thiothixene.
