Medicamentos anti-inflamatorios no esteroideos (Aine) puede causar daño de la mucosa gastrointestinal, el riesgo de que parece estar relacionada tanto con la dosis y duración de la terapia. Seria GI toxicidad, tales como sangrado, ulceración y perforación puede desarrollar en cualquier momento, con o sin síntomas de advertencia, y se produce en aproximadamente el 1% de los pacientes tratados durante 3 a 6 meses y de 2% a 4% de los pacientes tratados durante un año. Estas tendencias continúan con la duración de uso, aunque a corto plazo de la terapia no está exento de riesgos. Mientras que los agentes que inhiben selectivamente la ciclooxigenasa-2 (es decir, los inhibidores COX-2) se piensa generalmente para ser asociado con un menor riesgo de toxicidad GI en comparación con el convencional Aines, no se han demostrado libre de riesgo. Además, hay evidencia de que los inhibidores COX-2 pueden retrasar la curación de las úlceras gástricas, y probablemente la misma medida que los Aine tradicionales. Siempre que sea posible, especialmente si el uso prolongado se prevé, el tratamiento con no ulcerogenic agentes debe ser el primer intento. Si los AINES se utilizan, los pacientes deben ser tratados con la menor dosis efectiva para la duración más corta posible, y el tratamiento profiláctico con un agente citoprotector (por ejemplo, misoprostol), H2 de la histamina antagonista de los receptores, o un inhibidor de bomba de protones debe ser administrado como sea necesario.
