El uso de inhibidores de la HMG-CoA reductasa está contraindicado en pacientes con enfermedad hepática activa o inexplicable, elevaciones persistentes de las transaminasas séricas. Inhibidores de la HMG-CoA reductasa se metaboliza extensamente por el hígado. Disminución del metabolismo de los fármacos puede conducir a la acumulación y aumento del riesgo de toxicidad, incluyendo anormalidades bioquímicas de función hepática y, rara vez, ictericia, hepatitis, cirrosis, esteatosis cambio en el hígado, y necrosis hepática fulminante. La terapia con inhibidores de la HMG-CoA reductasa debe ser administrado con precaución en pacientes con antecedentes de enfermedad hepática y/o el uso excesivo de alcohol. Una menor dosis inicial puede ser apropiado, y la vigilancia clínica de los niveles de transaminasas hepáticas según el paquete del producto etiquetado se recomienda. Los pacientes que desarrollan elevación de ALT o AST niveles durante la terapia deben ser monitoreados hasta que las anomalías resolver. Si un aumento por encima de 3 veces el límite superior de lo normal persiste, debe considerarse una reducción de la dosis o la suspensión de la terapia.
